(e.g., allied health services)\u00a0for 12 weeks.<\/li><li>End-of-life pathway to boost services for people diagnosed with 3 months or less to live to enable them to remain at home, surrounded by those they love and to end their life with dignity and support.<\/li><li><span style=\"color: #000000;\"><strong>Assistive Technology and Home Modifications (AT-HM) Scheme<\/strong><\/span>\u00a0funded separately to ongoing participant budgets\u00a0and provides access to items used to assist a person to remain safely at home and live a fuller life such as
